OSHA Report Online a Fatality or Severe Injury By Melissa Esham 2/4/2016 OSHA requires that employers notify them when an employee is killed or suffers a work-related hospitalization, amputation, or loss of an eye. Fatalities must be reported in 8 hours and all other accidents resulting in hospitalization within 24 hours. You can call the 24-hour hotline – 1-800-321-6742 Find your local OSHA office Or Report Online Maryland Employers Please note MOSH Fatality Hotline is 1-888-257-6674. To report in-patient hospitalization, amputation, or loss of an eye please call MOSH at 1-800-759-6297 MOSH (Maryland Occupational Safety and Health Administration) does not allow online reporting. Virginia Employers Virginia requires that all fatalities and hospitalizations, amputations or loss of eyes be reported in 8 hours! This is more stringent then the federal requirement.
